HOUSING CASE MANAGER

Job Status: Permanent

Hours of Work: 21 hours per week, 0.6 FTE

Base office: Sundridge

Purpose of the Position: The Housing Case Manager is responsible for carrying out case management, psychosocial rehabilitation, crisis intervention and treatment for individuals living with serious and persistent mental illness under the clinical supervision of the Crisis Team Leader or designate. This team member has the clinical skills and experience necessary to assess, plan, develop, co-ordinate, and provide treatment by way of rehabilitative and supportive services in a multi-disciplinary environment. The Housing Case Manager develops supports and opportunities for consumers to increase or develop new skills needed to support success and satisfaction in their daily living, learning, working and social environments of their choice with the least amount of professional intervention required. This team member will be an expert in engaging the community and developing a network of landlords, community service providers and consumers in the District of Parry Sound. This team member will work with the end goal of decreasing stigma associated with mental illness, addictions, homelessness and poverty through the development and delivery of education and support to the community and the established network. This network will be promoted as a sustainable resource and as a strategy to decrease homelessness in the District of Parry Sound.

Requirements:
Requirements of the Position:

• A Bachelor’s degree in a behavioral science or equivalent (equivalency for the purpose of qualification means a university education in the social sciences, which includes or is supplemented by practical courses in these disciplines)
• Registration, membership or application for membership in the college appropriate to your discipline is an asset
• Experience providing case management services to those experiencing serious mental illness/addictions
• Familiarity with Psychosocial Rehabilitation principles and practices and the concept of recovery
• An understanding of the rental market, homelessness and poverty issues in the District of Parry Sound
• Working knowledge of the Residential Tenancy Act (RTA)
• A commitment to the rights of those experiencing serious mental illness/addictions
• Knowledge and awareness of Indigenous culture and language considered an asset
• Ability to work autonomously as well as cooperatively within a team environment in a rural setting
• Excellent communication, organization and interpersonal skills
• Working knowledge of Microsoft Windows, the Internet, and data management software
• A willingness to serve clients within the communities of the District of Parry Sound
• A valid Ontario G or G2 driver’s license and a safe and reliable means of transportation
• Willingness to work days/evenings (including weekends and statutory holidays)
